 Action Distorts hyphae and inhibits mycelial growth in susceptible fungi.

 Indications Treatment and prophylaxis of tinea pedia (athlete’s foot); treatment of tinea cruris (jock itch) or tinea corporis (ringworm) caused by specific fungi; treatment of onchomycosis, chronic scalp infections, palm and sole infections with kerion formation; treatment of tinea versicolor.

 Route/Dosage

ADULTS & CHILDREN: Topical Apply small amount of ointment, cream or powder or 1–3 gtt of solution to affected area bid for 2–3 wk (6 wk if skin is thickened); continue treatment to maintain remission. Reserve powder for mild infections.

DERM: Sensitization; mild irritation; pruritus; contact dermatitis; tinging.

PATIENT CARE CONSIDERATIONS

 Administration/Storage

  • Cleanse skin with soap and water and dry thoroughly before applying.
  • Apply small quantities of ointment, cream, solution, spray liquid, gel as primary therapy.
  • Use powder or powder aerosol as adjunctive therapy or prophylaxis for athlete’s foot. Use as primary therapy only in very mild conditions.
  • Consult physician if no improvement after 10 days.
  • To prevent recurrence, continue treatment for 2 wk beyond disappearance of symptoms.
  • Store at room temperature. Avoid moisture and sunlight. Keep powders from getting wet or damp.

 Assessment/Interventions

  • Obtain patient history, including drug history and any known allergies.
  • Monitor for sensitization, irritation, pruritus, rash, stinging.

 Patient/Family Education

  • Tell patient to cleanse skin with soap and water and dry thoroughly before applying.
  • Emphasize to use only externally in small amounts massaged well into affected area and surrounding skin.
  • Instruct patient to wash hands before and after applying medication.
  • Tell patient to avoid contact with eyes.
  • Advise patient to wear well-fitting, ventilated shoes and to change shoes and socks at least daily.
  • Tell patient not to cover treated site with bandages unless ordered specifically by physician.
  • Instruct patient to notify physician if no apparent improvement after 10 days of treatment.
  • Emphasize that product should not be used on nail and scalp infections unless using in addition to internal medication prescribed by physician.
  • Tell patient to store at room temperature and not in bathroom (specially powders).
  • Instruct patient to report these symptoms to physician: persistent or severe irritation, itching, rash, stinging.
